Patient Forms
If you are a new patient to our office, please visit our Patient Registration page to complete forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless prior financial arrangements have been made with the office staff. If you have questions regarding your statement of account, please contact us at 703-689-2697. Monthly statements are sent for accounts with a pending balance. If you have questions about your bill, please call us at 703-689-2697. We’re happy to help — often, a quick call resolves any questions.
As a courtesy, we will bill your insurance and track claims. However, you are fully responsible for all fees charged by our office regardless of insurance coverage. Your prompt payment is greatly appreciated.
Cancellation Policy
If you need to cancel or reschedule an appointment, we ask that you provide at least 24 hours’ notice. A cancellation fee may apply if adequate notice is not received.

We are happy to file insurance claims on your behalf. Please bring your insurance card and any necessary information with you to your first visit.
Important Notes:
Insurance policies typically cover only a portion of treatment costs.
Unless other arrangements have been made, your estimated portion is due at the time of service.
You are responsible for any balance not paid by your insurance provider.
